German translation

Correct, O'Reilly went and did their own private German translation;
we don't have access to those sources.  It's essentially a proprietary
fork of the book.  We were also quite unhappy that the German division
of O'Reilly put the words "2nd edition" on the book, when in fact the
real 2nd edition is what we're working on now.

We'd definitely welcome contributions to the open-source translation!
